Key features

The weapon system is a missing arm

The doll has no right arm, and you craft what goes there: an enchanted candelabrum, a chain rock, a machinegun, a flamethrower. Building weapons into a companion rather than a player character is unusual, and it means loadout changes read as changes to a character you are travelling with rather than a menu.

The map expands by killing what is holding it shut

Perimeter guardians gate the world, and defeating them opens more of it. That replaces the usual key-and-door structure with a difficulty check, so how far you can travel is a direct statement of how good you have become.

Every area has a second, worse version at night

Returning to a cleared area after dark produces new creatures and new secrets. This is the mechanic that gives a horror RPG a reason to backtrack — revisiting is not a chore for a missed item, it is a different level.
